1. Assumption of Risk

I, the undersigned participant (or parent/legal guardian if participant is under 18), understand and acknowledge that participation in activities organized by Dale Futbol Club LLC ("Dale FC"), including but not limited to pickups, leagues, training, camps, and other related activities, inherently involves risks of physical injury, illness, permanent disability, property damage, and even death. I voluntarily assume all such risks, both known and unknown, whether caused by the negligence of Dale FC or otherwise.

2. Medical Authorization

In the event of an injury or emergency, I authorize Dale FC personnel, including staff, coaches, volunteers, or facility representatives, to seek and obtain medical treatment for me or my child. I understand and agree that I am financially responsible for any and all medical services rendered. Dale FC assumes no responsibility for medical care or related expenses.

3. Release and Waiver of Liability

In consideration for being permitted to participate in Dale FC activities, I, on behalf of myself, my heirs, executors, administrators, assigns, and next of kin, hereby fully and forever waive, release, discharge, and covenant not to sue Dale FC, its members, officers, directors, employees, contractors, volunteers, and affiliated facilities from any and all claims, demands, liabilities, damages, or causes of action arising out of or relating to participation in its programs, whether arising from negligence, premises liability, or otherwise.

4. Indemnification and Hold Harmless

I agree to indemnify, defend, and hold harmless Dale FC and its agents from any and all claims, losses, liabilities, damages, or costs (including attorney's fees) that may arise from my or my child's participation, whether caused by negligence or otherwise, including third-party claims caused by actions or conduct during participation.

5. Photo, Video, and Media Release

I hereby grant Dale FC the unrestricted right to record and use photographs, video, or audio recordings taken during activities for marketing, promotional, or educational purposes, without compensation or approval. This includes publication on social media, websites, printed materials, and other platforms.

6. Behavior & Conduct Policy

I acknowledge that Dale FC reserves the right to remove any participant who violates behavioral expectations, facility rules, or engages in conduct that poses risk to others or disrupts programming. Removal will be at the sole discretion of Dale FC staff and without refund.

7. Severability and Governing Law

If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect. This agreement shall be governed by the laws of the State of Maryland, and any legal action shall be brought exclusively in the appropriate courts of that state.

8. Binding Agreement

I acknowledge that I have read this waiver in its entirety and understand its contents. I voluntarily agree to be bound by its terms. I understand that by signing below, I am giving up substantial legal rights.
